Substitute Intent Information

This form is intended to be completed by SBHC contracted employees who are resigning, retiring, or non-renewed. By completing and submitting this form you are requesting to be added to the active substitute pool.

 

The process is as follows:
  1. Click here to complete the Substitute Intent portion of the electronic form. You must complete and submit this form PRIOR TO YOUR LAST DAY WORKED. Forms received after this date will be considered a break-in-service and will require re-drug testing and re-finger printing at your expense.

  2. Once the form is submitted it will be reviewed by Human Resources to verify that all minimum substitute requirements have been met. At this point, if any further action needs to be taken, you will be contacted by an HR staff member.

  3. Upon final approval of the Substitute Intent form you will receive an email confirmation (You must use your personal email address). Please be advised this process may take several weeks.

  4. View the SmartFindExpress substitute presentation, and print the substitute quick reference card for instructions on how to use the automated system.
