Leave of Absence Procedures

 

LOA Types

 


Employee Procedures for Leave of Absence (LOA)
  1. Complete MIS form 09.11 - LOA Notification.
  2.  
  3. Complete MIS form 09.78 - Application for Family Medical Leave.
  4.  
  5. Provide a doctor’s note with beginning and ending dates for your medical leave of absence.
  6.  
  7. Return doctor’s note (if medical leave), LOA form 09.11 and FMLA application form 09.78 to your supervisor, manager or bookkeeper. The beginning and ending dates on the doctor’s note, LOA notification form, and FMLA form must match.

  9. EPAF processor and/or bookkeeper will forward to the Human Resources Department.

  10. *** MIS form 09.49 may be given to your Medical Physician if their office does not provide a doctor’s note. ***

Employees who apply for leave under the FMLA guidelines may be eligible if they have worked for at least 12 months, and worked for 1,250 hours over the previous 12 months. All Employees on leave under FMLA guidelines may continue to receive School Board insurance benefits for the duration of the approved leave (up to 12 weeks). If you do not qualify for FMLA benefits then you are responsible for your insurance premiums. For information please call Tammy Mortensen, 471-5669 for all insurance questions before you apply for any leave.
