Sick Leave Pool

The Sick Leave Pool Program is designed to assist employees who are required to be out of work for an extended period of time due to personal medical reasons and have exhausted all of their sick leave. However, there is a limit not to exceed 40 days from the Sick Leave Pool for any one illness or injury or complications thereof.

 

To join the Sick Leave Pool:
There are two windows during the year in which you may opt to join the pool. A completed application may be turned in starting during the last week in August or the first week in February for a period of two (2) weeks.
To qualify for the Sick Leave Pool Program you must be a Sick Leave Pool member, use all sick & personal days on the books, go five (5) days leave without pay and need a minimum of five (5) days from the Sick Leave Bank. Please remember to contact your bookkeeper and provide all forms and requirements including a Family Medical Leave Form so that you are an approved leave of absence. Once you feel you have met the qualification of draw from the Sick Pool program please contact the benefits department (471-5669) or download the Sick Leave Withdrawal Form and your doctor must sign a Physician’s Statement.
 

 

Requirements to draw from the Sick Leave Pool:
  1. Join the Sick Leave program during one of the two week open enrollment windows in August and February and complete a 30 day waiting period.
  2. Be placed on leave by a physician, exhaust all sick/personal leave, go five (5) days leave without pay and need at least an additional 5 days from the Sick Leave Pool.
  3. Download or request a Sick Leave Withdrawal Form and your doctor must sign a Physician’s Statement. Complete all requirements on both forms and return to the Insurance & Benefits Department.
  4. Complete all paperwork with your bookkeeper to be placed on approved leave with the SBHC.

However, if you have any questions, concerns or need paper copies of the forms, you can call the Insurance Benefits Office at (863) 471-5669 or pick up the forms at the District Office located at 426 School Street, Sebring Florida. Office hours are 7:30 a.m.- 4:30 p.m.
